We had some of the same concerns as Anne. However, we have found that 9 times out of 10 the item that says it is in transit for longer than a day is sitting on the shelf where it should be. We figured in the 1 in 10 case where it was not if a customer was looking for it and could not find it they would notify us and we would initiate our searches process. We have promoted with our customers that if they can not find something to come let us know so we get good notifcation on these. Also we now have an Express Retrieval Service where we will pull an item and place it on the hold shelf at the owning library. If something is "missing" that service will initiate the searches process.
We also don't have the person power to hunt down the in transits that do not clear on a regular basis.

Personally, I wouldn't like this. Having the In-Transit manually cleared at
the location it belongs helps me to be sure that items are returned properly
to the correct branch/location. If a Transit does not clear, then I know it
is an item to search. I frequently find "Missing" items this way, and the
transit note not cleared gives me a great clue where to look, since it shows
the last check-in location.
