[IUG] Suggest a Purchase Form

Hello:

We are trying to customize the Suggest A Purchase form (acquire) by adding more fields. We want it to link into the Recommend Mode in Milacq with the additional information. We have found the instructions in the III manual, but we still cannot get it to work.  I would deeply appreciate advice and examples from libraries that have successfully customized this form.
