Summary of responses to question about authors' names in tables of contents

Happy New Year, everyone!

Eight people kindly responded to my question about controlling authors' 
names in TOC entries. Most feel the same frustration we feel here. The 
majority do no regular authority control of these names unless a problem 
is called to their attention. Two libraries have actually had the 
author's names removed from the author index, though not without 
reservations.

So I still have no magic bullet (in fact I don't think there is one!), 
but at least I am not alone! Thanks for all your help!
